New Patients
Please download our patient forms, fill them out as completely as possible and bring them with you to your appointment. For medical appointments, please bring the following (if applicable):
- Photo ID
- Insurance card
- Primary physician referral (when required)
- A list of any medications and / or eye drops you are currently using along with your eyeglasses / or contact lenses
- If consulting with Dr. Pacheco regarding eyelid or eyebrow procedures, please bring an old photograph of yourself (an 8 x 10 high school graduation picture is ideal).
